Supporting Integrated Stroke Care Transitions: An Interprofessional Learning Simulation

Description: This presentation describes the development of a video-recorded, actor simulation aimed at developing quality and safety competencies for interprofessional integrated stroke care. The simulation was co-developed with a team of academics from Toronto Metropolitan University and expert stroke clinicians from Sunnybrook Health Sciences Centre. The simulation focuses on the roles of the team members in the context of an interprofessional integrated stroke care team coordinating the care of an older adult living with stroke and other comorbidities. The scenario evolves through two scenes which includes two care transitions: 1) acute care hospital to rehab hospital; and 2) rehab hospital back to home in the community. We will describe how the simulation can be used as an interprofessional education learning resource in academic and practice settings.
